Enrollment/Registration Process

 

Steps for Enrollment:

 

A completed application, forms, records, birth certificate, and non-refundable deposit are required in order for registration to be complete. An interview is also required of all new families, and ALL STUDENTS MUST HAVE A CURRENT IMMUNIZATION RECORD UPON REGISTRATION.

 

Steps for Enrollment:

 

1. Complete an enrollment application in the office each year.  To complete your child's application the first time, you must bring the following information to the elementary office

 

• Immunization Record (DHEC Copy)

 

• Three (3) doses or combination of DTP, DT, DTP - Hib, DYaP, or Td vaccine with at least one (1) dose received on or after the fourth birthday.

 

• Three (3) doses of any combination of oral or inactivated Polio vaccine with at least one (1) dosed received on or after the fourth birthday.

 

• One dose of Rubella (Measles) vaccine received on or after the first birthday, except that children admitted to any child development program under the control of the State Department of Education, or to kindergarten through fifth grades, must have two (2) doses of Rubella (Measles) vaccine with both doses received on or after the first birthday and separated by at least one month.

 

• One (1) dose of Rubella (German Measles) vaccine received on or after the first birthday.

 

• One (1) dose of Mumps vaccine received on or after the first birthday.

 

• Three (3) doses of Hepatitis B vaccine for all children admitted to kindergarten, first, second grades.
